Please complete this form to request an ASHE member dues voucher awarded as part of the ASHE chapter award program. Before completing this form, please make sure you:

Are a new ASHE Member
Are a first-time voucher recipient

Individuals who do not meet the above requirements will not receive a voucher.

Once you complete and submit this form, an ASHE staff member will connect with you within 5-10 business days.

All voucher requests are due by October 31, 2025. Requests received after October 31 will not be fulfilled.
