A&F Division Awards Nomination Form

ELIGIBILITY

All active division employees (full-time and part-time staff and administrators) may submit a nomination, and are eligible to win an award. Teams are, in general, more than one, but fewer than 10 people.

AWARD CATEGORIES AND CRITERIA

• DEI Champion Award – This award acknowledges an employee who demonstrates commitment to the inclusion of all perspectives, someone who promotes/develops/considers processes that are considerate of the campus’s diverse background and actively works to remove barriers to success. This employee is someone who celebrates/brings awareness to traditions/experiences that celebrates our diverse culture.

• Teamwork Award – This award celebrates employees who have supported colleagues in meeting goals, deadlines, and responsibilities, who have coordinated cross-divisionally or with other A&F departments to develop solutions or collaborate on a project or process that best serves our campus community. Nominees for this award promote a collaborative work environment and value the perspectives of others.

• Outstanding Service Award - This award recognizes employees who consistently deliver exceptional customer service to the campus community and going above and beyond their regular duties. Nominees for this award are known to exceed expectations.

• Division MVP Award – This accolade acknowledges an employee’s exemplary effort, achievements, and contributions that align with the division’s values, mission, and goals. Nominees for this award are positive role models for their colleagues.

• Exceptional Work Commendations (primarily CSUF PD) - lifesaving commendations, safety accomplishments.
