Sexual Harassment (Title IX) Reporting Form

Thank you for promptly contacting the Title IX office to report sexual harassment.


Completing this form fulfills your sexual harassment reporting requirements pursuant to District Policy AR 5145.7. Not all types of sexual harassment or misconduct fall under the Title IX regulations, nor do they require immediate intervention from the Title IX Coordinator. However, it is your responsibility to report all allegations of sexual misconduct to the Title IX office.

Immediate Response Steps


1) If there is an immediate threat or safety concern, contact SDUSD Police at (619) 291-7678.


2) Ensure the safety and well-being of the students involved.

  • Implement a Safety Plan and/or Supportive Measures.

  • Contact the respective parents to notify them of the concerns and discuss the safety plan/supportive measures.
  • At this time, this incident is not under a Title IX investigation (pending Title IX assessment).
  • You can advise the alleged victim that they have a right to file a formal Title IX complaint with the Title IX Office. You can provide them with the Title IX office contact information.

                                            

3) Conduct a minimal facts inquiry (not a formal investigation).

  • Gather information so that the Title IX office can determine next steps. For example, witness statements, video footage and other relevant evidence.
  • Try to minimize the number of times an alleged victim is interviewed.


4) You cannot take disciplinary action.

  • If it's not safe for the alleged respondent to be on site, you can implement any or all of the following:

                     (1) Title IX Emergency Removal   

                     (2) An independent study contract, or

                     (3) other non-disciplinary measures.


5) Additional reporting requirements.

  • Mandated Reporting to Child Welfare Services for abuse/neglect.
  • SDUSD Police (sexual assault/battery).


Following the Immediate Response Steps above and completing this form will ensure the District is in compliance with District policies and federal regulations.


The Title IX Officer will review the information immediately to determine next steps. The Title IX officer or another member of the ICA team will be in contact as soon as possible.
