By providing the information above, submitting a photo and providing your email address you will agree to the following multimedia release. With the goal of helping the EPILEPSY FOUNDATION to deliver education and information on epilepsy to the general public, I hereby grant to EPILEPSY FOUNDATION and its affiliates, successors, and assigns, with the right to grant to such other persons as EPILEPSY FOUNDATION may designate from time to time (collectively, the “EF Entities”), the absolute and irrevocable right to take, use, reproduce, publish, distribute, perform, display, and/or copyright any photographs, motion pictures, audio, visual, or written recordings, or any other record of or including me, my image, name, voice, likeness, and/or other indicia of my identity or personal experiences, in any medium now known or hereafter developed, alone or in conjunction with other material, without restriction as to changes or alterations (hereinafter, collectively, the “Content”), for editorial, educational, promotional, advertising, and commercial purposes, including without limitation in connection with the solicitation of contributions and the furtherance of the corporate objectives of EPILEPSY FOUNDATION, in the United States or elsewhere in the world at any time in perpetuity.

I agree that any such Content, including any copies, derivatives, modifications, or alterations thereof, together with all copyrights and other intellectual property rights therein and thereto, is or will be owned by the EF Entities. If I should receive any Content from the EF Entities, I shall not authorize its use by anyone else for any purpose.

I hereby waive any right to inspect or approve the Content or the use to which it may be applied.

I hereby release, on behalf of myself and each of my beneficiaries and assigns, each of the EF Entities, and all persons acting with their permission or authority or those for whom they are acting, from any claims and liability to me, including all claims of libel, defamation, invasion of privacy, violation of right of publicity, infringement of copyright, or violation of any other right arising out of any use of my image, name, voice, likeness, and/or other indicia of my identity or personal experiences as contemplated herein.

I further agree that I will not hold the EF Entities, or anyone who receives permission from the EF Entities, responsible for any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form, whether intentional or otherwise, that may occur or be produced in the capturing of the Content or in any subsequent processing or publication thereof, even though it may subject me to ridicule, reproach, scorn and indignity.

[Other than any monetary compensation I may have already received,] I acknowledge that I will not receive [any other] monetary compensation for the use of my image, name, voice, likeness, and/or other indicia of my identity or personal experiences. I further acknowledge that the consideration for my agreement herein is the personal satisfaction I receive from my support of EPILEPSY FOUNDATION and its mission.
