College Report Request Form
The Common App College Report is processed by the Foothill Admissions & Records (A&R) Office. All requests will be emailed from Foothill to the college/university.
When completing this form, students have the option to request the College Report be emailed back to them OR students can provide an email of where they would like Foothill to email the College Report.
Universities NOT using the Common App may require students to email documents similar to the College Report. Sometimes this document is called a “Transfer Report” or “College Officials/Admissions Report.” Convert the document to a fillable PDF, fill out your section, and use this College Report Request Form to request Foothill (A&R) to fill out their section and email it for you.
