Type of Request

Check this box if data will be communicated outside of the Dish or School Nutrition website and you need additional communications support. Please explain intended channel and details on how the communications team can assist in the box below. Examples of areas for communications support might include, but are not limited to, interview prep, messaging, resource development, CDE-wide channel submissions, etc.

Recurrence*

Provide information on the intended use of the data, what is the request for? What question(s) are you aiming to answer and/or what you trying to measure with this data?

How will this data be shared? Who is the intended audience or stakeholders to which this data will be presented?

Provide a detailed description of the request including all pertinent information. This must include which School Nutrition Programs, years (program year and/or school year), and sponsor or site level data to be provided. Please add as much additional detail as possible/needed to further define your request.

Depending on the complexity of your request and the capacity of data team, please allow for at least 10 business days whenever possible.

Note that this does not guarantee your request will be completed any sooner than the 10 day timeframe, but will alert us to its urgency and we will do our best to prioritize accordingly.

Please attach anything that will help us with your request, including screenshots, request forms, etc.
