Northwestern School of Communication

Office of Undergraduate Programs and Advising

Please use this form to petition for an independent study in the School of Communication. This form applies to independent studies in the following departments: Communication Studies, Communication Sciences and Disorders, Performance Studies, Radio/Television/Film, Theatre, and Dance.

Once filled out, this form will be routed to your faculty sponsor for approval. For theatre majors, it will also be sent to your department chair. All fields are required.

Once approved, you will be registered for the appropriate course by the Office of Undergraduate Programs and Advising and the relevant department. If there is a problem, the office will contact you.
