I declare; that to the best of my knowledge, all the information I have provided on this form is accurate and true.
• I agree to; abide by the policies, procedures & directions of MoWSA, provide information to conduct a National Police Check every 3 years, Supply an independent reference if required and that I will sign the ‘fit for work and driver declarations’ at my branch when I am rostered, to have my image or name used for purposes of promoting MoWSA through newsletters, newspapers, electronic, social & other mediums and my details noted above, may be used by MoWSA to send me information about my role as a volunteer, e newsletters, updates, & other information that may be of interest to me as a volunteer. I know I have the option to unsubscribe if I so desire.
• I understand that;
• It is my obligation and responsibility to MoWSA, its customers, volunteers and staff, not to disclose any confidential information obtained in the course of my duties.
• MoWSA will only use or disclose personal information for the purpose for which it was collected, or in circumstances related to public interest such as law enforcement & public or individual health and safety. E.g. Use of emergency contact details, if needed, sending you correspondence.
• That all new volunteers are required to commit to at least 3 months initial service, which will also serve as a probationary period. This, so that we may complete our Induction Program with you, give you sufficient time to gain enough information to ensure you understand the roles, structures and policies of Meals on Wheels SA. And emphasising your rights and obligations as a volunteer. It also serves as an important part of the recruitment and selection process. This period is used for the continuing evaluation and assessment of new volunteers suitability, and for both MoWSA and the volunteer to assess a number of factors, including satisfaction of the role, behaviour, culture, fit and that the role meets your expectations.
