This form is now closed. OHA's inventory of self-tests has been completely depleted, and we are no longer able to provide these supplies.

Point-of-care testing supplies for use in clinical settings under CLIA guidance will remain available and can be requested through this form. Organizations must be able to provide a valid CLIA number to receive point-of-care testing supplies.
