See it, Hear it, Report it - Match Official

This form is to be used to report a Match Official's poor performance and/or poor conduct. The information you provide will be investigated by the West Riding FA Referees Development Team. Any outcome resulting from the investigation will be managed exclusively between the Referees Development Team and the Match Official concerned. If the report relates to a disciplinary or safeguarding matter, you will be asked for permission to disclose your evidence as part of any charge proceedings. All other matters outside of discipline or safeguarding will not require you to disclose your personal details.
