Course Development Request Form

This request form is exclusively for Deans, Chairs, and Directors, who can make requests for new course development or major course revisions.

Please justify your request by demonstrating the need for a new course or major course revisions. Also, if you are requesting a major course revision, please have the developing faculty member complete the following Course Revision Planning Form to outline the necessary course revisions and attach it at the bottom of this request form.
