KEY/FOB REQUEST FORM
This form is to be used for both campus building access, and for individual door schedule requests. The form must be submitted before any access or openings will be completed. All requests will be acknowledged by a reply email, so please fill in every field and verify your contact information.
The key holder’s Hartnell ID number is required for issuance of a key. If the individual does not have a Hartnell ID, a key will not be issued. Any and all exceptions must be approved by Vice President of Administrative Services in advance. A Supervising Department Employee must take responsibility for key issued to non-Hartnell personnel (i.e. Volunteers, Contractors, Student Assistants, etc.). The information of the Hartnell employee taking responsibility for the non-Hartnell Personnel is required.

*If your key is lost or stolen, please contact Campus Security at (831) 755-6888. Please provide your name, phone number, your ID number, and the date and time when you noticed that your card was missing.
There is a replacement fee for lost or stolen keys.
