Rotation Evaluation

This form must be filled-out by the Faculty Rotation Mentor immediately after completion of rotation. The Rotation Mentor should provide feedback and approval of the Rotation Prior to completing this form. Feedback should be given in person by the Rotation Mentor to the student once it is filled out. Once completed, this form will be automatically routed to the mentor, student, and program director for approval. Student Learning Outcomes

The following Student Learning Outcomes (SLOs) are foundational to the BSGP. Select the level of development for each skill set during the rotation, with 1 being the lowest level and 5 being the highest. A 5 represents an accomplished scientist (about to graduate with an MS or PhD). First-year graduate students are often in the 1-3 range. Scores can be used to identify growth during the rotation and areas for needed future growth.
