Locker Rental Request Form
$8.00 per term, per locker will be charged to your student account each term.
Please list the minimum length, width, and height required for your instrument, case, and any accessories.
Please list the music course/ensemble for which you are registered. You must be registered for a music course/ensemble in order to rent a locker.

Please provide your preferred location and include any necessary accommodations for your locker placement.
Please note that the School of Music and Dance (SOMD) cannot guarantee the security of your personal instruments or other items left in the SOMD lockers, rehearsal rooms, classrooms, or any other location in the school. Because our insurance does not extend to your personal property, we must refuse responsibility for any theft, vandalism, or other loss to your belongings that might occur on SOMD property. As a convenience to you, we allow you to store your instruments temporarily at the SOMD for a small fee. While we attempt to keep the building secure, you should be aware that damage and thefts do occur. We encourage you to buy personal insurance coverage for your instruments and other valuable items you bring into the School of Music and Dance. This coverage can usually be obtained as part of your homeowner's insurance.
