What are degree audit forms?

  • The degree audit forms are intended for Skyline College Counselors.
  • The purpose of the form(s) provides supplemental information about your student's degree/certificate and aid in the evaluation process.
  • The counselor will coordinate with division deans should addt'l signatures be required.

Degree Audit Forms

You may upload as many degree audit forms for the major being evaluated. Please submit a separate request if there is more than one major being requested for evaluation.

  2. Please include the pdf copy of the student's Degree Works worksheet as a reference for the evaluation, and
  3. Please include the supporting docs used to determine your student's eligibility for the degree/certificate. Examples:
