Request for Advisory Committee

This form is required for all School of Medicine graduate students enrolled in a PhD or master's thesis degree program. Advisory committees guide and advise student research and dissertation/thesis preparation.

PhD Students: the committee must be appointed within six months after Advancement to PhD Candidacy or upon commencement of dissertation research, whichever is earlier.

Master's Students: the committee must be appointed within nine (9) months of matriculating into the graduate program. Programs may require appointment of the committee earlier than the minimum guideline.

Programs may require appointment of the committee earlier than the minimum guideline. Students may request permission to replace one committee member by a recognized scholar who is not a member of the Graduate Faculty. These requests must include a short letter from the Program Director and NIH-style biosketch for the proposed member(s). It is the student and programs responsibility to ensure that committee members meet all requirements prior to submission of this form.
