Assessment/Testing/Decommission

The purpose of the clearance assessment/testing/decommissioning procedures is to minimize the potential health and safety risk to :

•   the public (movers, salvage equipment buyers, etc.) who may have direct contact with items being decommissioned;

•   the environment;

•   personnel involved with the renovation/demolition/construction;

•   future users and occupants.

   

These procedures apply to persons who are responsible for, or have oversight of potentially contaminated equipment, a laboratory, or laboratory equipment that is/are being decommissioned when:

•   Leaving the university and closing a laboratory (or area where hazardous/regulated materials were used)

•   Retiring and closing his/her laboratory (or area where hazardous/regulated materials were used)

•   Relocating their operations to a different building on campus

•   Leaving the university but transferring responsibility of their laboratory /operations to another researcher

•   Disposing of potentially contaminated laboratory/non-laboratory equipment for salvage or waste

•   Preparing potentially contaminated laboratory/non-laboratory equipment for repair, or transfer to a new owner


Prior to clearance assessment, all accessible surfaces must be free of contamination. If biological agents have been in contact with the equipment and/or surface(s), disinfect with a suitable disinfectant (e.g., a 10% bleach solution freshly made), or in the case of Biosafety Cabinets, hire an NSF 49-certified vendor to perform the decontamination. If radioactive materials are or have been used, decontaminate the surface/item or contact the Radiation Safety group at EH&S for guidance. Contact EH&S if you need additional information about proper disinfection procedures.


Important Notice: EHS will acknowledge receipt of the request prior to the onsite assessment. Onsite clearance assessments are conducted on the LAST Wednesday and Thursday of the month. ASSESSMENT REQUESTS MUST BE RECEIVED AT LEAST ONE WEEK BEFORE THE ONSITE DATES, AT THE LATEST. Requests that are received after this deadline will be placed in the queue for the following month. Please plan your needs according to this schedule.

Special Note:


• Items in freezers can remain as long as the contents are completely frozen and secure (from movement), AND the freezer will only move WITHIN the campus grounds.


• FM movers are not authorized to move/relocate any liquid, chemical, any powdery substance or hazardous materials in the lab, All glassware and containers must be completely dry and empty.


• If applicable, all equipment must be disconnected from all lines (electrical, water, air, etc.) prior to the move. FM movers are NOT authorized to do any disconnecting/connecting.

Biological / Chemical / Radioactive Material Status

All applicable statements must be checked "Yes" to indicate completion of the action in compliance with the requirements; or if the statement is not applicable, choose "N/A".

I/We removed all cultures and stocks of microorganisms and regulated medical waste from the premises.*
Radioactive materials were used with this equipment, or in the lab.*
The area/item(s) is/are free from radiation contamination.*
I/We cleaned, drained and decontaminated contamination-prone equipment (refrigerators, centrifuges, water baths, GC, HPLC, incubators etc.) that will be removed from the laboratory.*
I/We removed chemicals requiring disposal as hazardous waste.*
I/We guarantee that all benches have been cleaned and decontaminated.*
I/We guarantee that the fume hood has been cleaned and decontaminated.*

Biosafety Cabinet Clearance Request

Please choose ONE of the following options:

This option does NOT require decontamination by an NSF 49-certified contractor/vendor. Nonetheless, cleaning and decontamination must still be performed.

Method used for decontamination:

This option REQUIRES decontamination by an NSF 49-certified contractor/vendor.
