By completing this form, I understand that OPTAVIA will notify all parties of the Request. I also understand that the Client reserves the right to select a Coach of their choosing. Requests are kept on file for record-keeping and audit purposes. Transfers are typically processed in 1-3 business days and, once approved, will be made effective the date the Client Transfer Request was received by OPTAVIA. Any and all volume that appeared on the Client’s account will not move with the Client, but will remain with the previous Coach. However, any new orders placed by the Client after the Transfer Request has been received and processed, will be credited to the new Coach.
