WDRS Change Request Form

This form is used for change requests for general WDRS functions, including:


  • Adding and editing questions, reports, and workflows
  • Updating reference lists
  • Adding rosters
  • Database update requests
  • Other requests related to WDRS software


Please send questions or concerns to WDRSDevs@doh.wa.gov for the following:


  • User access
  • Requests to open cases
  • Inquiries about WDRS system performance that do not require a change request
  • Large projects requiring multiple change requests


Upon completion of this form, you will receive automatic notifications from Smartsheets with status updates.

Please rate the level of perceived impact this change request would have on daily operations:*

A: Critical - Daily business operations cannot proceed without functional resolution, immediate attention and solution required. (IE work stopper/ work blocker)


B: High - System functionality does not exist, there is an time sensitive need to develop a solution.


C: Medium-High - System functionality does not exist but a reasonable workaround does ( this is not time sensitive).


D: Medium-Low- System functionality exists but should be refined.


E: Low- Functionality that bears no impact on business outcomes but would remedy a functional error ( i.e. typo, extra step etc).
