WDRS Change Request Form
This form is used for change requests for general WDRS functions, including:
- Adding and editing questions, reports, and workflows
- Updating reference lists
- Adding rosters
- Database update requests
- Other requests related to WDRS software
Please send questions or concerns to WDRSDevs@doh.wa.gov for the following:
- User access
- Requests to open cases
- Inquiries about WDRS system performance that do not require a change request
- Large projects requiring multiple change requests
