Submit a Resource for the CardiQ Resource Library

Use this form to submit files for the Resource Library on CardiQ. Please only submit one resource per form. Submit materials which you are comfortable sharing on the website. We are looking for checklists, QI related research, templates from your own QI projects, protocols, business plans, teaching materials (for patients and staff), guidelines, white papers, etc.


Tagging Your Resource: Remember to tag your resource to the appropriate tags so that CardiQ users can easily find your materials. There are 5 tag categories. You may have more than one tag per category.

  • In submitting materials, I am allowing HRS/CardiQ to publish said materials for defined purposes while retaining overall ownership and creative license, if owned by the contributor. I am aware that materials submitted will be viewable and made available for download by the general public.
  • I understand that my submission will be reviewed by the CardiQ Content Curation Working Group, and may be accepted or rejected based on the above criteria. HRS/CardiQ reserves the right to remove accepted submissions at any time for any reason.
  • I agree to review any material submitted for relevance and accuracy on a yearly basis or when prompted by HRS/CardiQ.
