Portland Fire & Rescue Subpoena Submission Form

This form may be used to submit criminal and civil subpoenas for witness attendance, when the witness is a Portland Fire & Rescue employee acting in their capacity as a City employee, and criminal and civil subpoenas for documents (subpoena duces tecum).


PF&R requires reasonable time to process subpoenas, typically 14 days or within the statutory timeline, whichever is later. Subpoenas submitted with a production or appearance date sooner than 14 days might not be processed prior to the date, but PF&R does its best to timely fulfill subpoenas.


This form is NOT to be used for subpoenas related to the Fire/Arson Investigations Unit. Fire/Arson Investigators receive their subpoenas through the eCAT system by their assigned DPSST number.
