Nevada Committee (Local Planning Committee)
Thank you for your interest in being a vendor at the Nevada Department of Education's 11th Annual AI/AN Education Summit, on March 22nd & 23rd 2018 at The University of Nevada, Reno.
Please fill out the vendor form to identify what type of vendor you will be at this year's summit. Deadline to turn in your vendor form is March 9, 2018, no exceptions.

Indicate your vendor name or the name of your organization.
If you are the contact person you will receive all correspondence.
Tribal Affiliation or Organization:
Please describe your items in detail. If you have more than one item, please list your items in order of preference.
Terms of Application
This form, plus full payment, must be received by March 9, 2018. Full payment is due with return of this form. In addition to payment, the committee request one raffle items that is $15.00 or greater in value. Vendors may not set up until full payment is received. There are no refunds. Vendors are not permitted to relocated their booths, or sell outside of their assigned area. Subletting is prohibited. There will be no mobile sales (Carts, trays, etc.). * Only those items described and approved in this application shall be sold. Booths must be properly manned at all times. Vendor space is very limited, so reserve your space as soon as possible. The Committee will accept the first paid informational booths. No food or beverage sales allowed. * Vendors can begin setting up at 8 a.m. on Thursday, March 22nd, and must be taken down no later than 5 p.m. on Friday, March 23 Vendors will be provided one (1) table and two chairs. Due to limited space, only one (1) table per vendor is permitted.The committee will let you know as soon as they receive your payment. Vendors in violation of any of the above regulations will be asked to immediately cease selling, and leave the area. There are no exceptions to this stipulation. Please check the box below acknowledging that you accept the terms.
