Access to NITMB's suite in the John Hancock building requires two badges: a building badge issued by Hearn to access the elevators (blue badge) and a Northwestern University Wildcard ID (purple badge) to access the suite.

Northwestern University employees who are NITMB members can (1) activate their Wildcards to access the suite and (2) acquire a Hearn elevator badge by submitting below.

What you need to provide:

Complete and upload this form with your submission. Please note: your signature must be written, not typed. You may sign electronically as long as it is written.
A 4x4 high-resolution headshot
please take the picture against a plain white background
please do not wear a hat or sunglasses
please submit the headshot as a jpeg
First and last name

Northwestern Wildcard ID # (seven digits, front of the card, across from "ID")

In gaining badge access to NITMB you agree to the code of conduct found at https://www.nitmb.org/code-of-conduct .

Timeline

Average processing time from receipt of completed badge form and headshot is five business days.

Access for on a current, non expired, Wildcard will takes around four days. Please note: university holidays will delay this timeline.

Who will contact me about access? Look for an automatic email titled "NITMB Access Badge(s) Ready for Pickup" from "Kierra Hannon via Smartsheet". We will reach out to you when we have secured your card(s) and they are ready for use.

Its the 7 digit number (no letters) on the front of your wildcard or found in the myHR system. Do not provide your netid, we are unable to add access with a netid.
