EPS Computer & Phone Order Form
Use this form to:
- Order a laptop, peripheral(s) or phone service for a new employee.
- Replace an existing laptop w/ an expired warranty or soon-to-expire warranty.
- Add, change or cancel Phone Service for the following,
- Desk Phone (Cisco 8851)
- Desk Phone w/ Camera (Cisco 8865)
- Cisco Softphone(WebEx).
EPS Subsidy: EPS provides a subsidy of $1700 that can be used towards purchasing a new computer. If your current computer is not covered under warranty or you are not a contingent employee, you are eligible for the subsidy. The remaining balance will be charged to your department's PTA.
Equipment Warranty: As per Stanford's policy, the warranty for all computers will be purchased and charged to your department. The estimated costs for the equipment and warranty can be found on the form, but they are subject to change without further notice.
It is important to remember that once your laptop's warranty has expired, you will no longer have access to IT support. In case of any technical issues, this could significantly affect your work. Therefore, investing in continued IT support is crucial to avoid warranty lapses and ensure continuous productivity and peace of mind.
One month before your laptop warranty expires, Operations will contact you to help you replace your laptop.
