1. Apply and Get Admitted: Team member and/or dependent has applied to and has been admitted to York College before submitting this application.
2. Complete Required Fields: Fill out all necessary fields for the benefit you are applying for.
3. Request a Copy: Before submitting the form, check the "Send me a copy of my responses" box. You will receive a copy of your submission via email for your records.
