This form should be completed by the department chair, dean's office, or UVA hiring official on behalf of the faculty member UVA wishes to recruit or retain.

Requests on Behalf of Current Faculty Members

For faculty members who began work more than one year ago, completion of this form is required for the Dual Career Program to initiate services with the faculty member's partner.

For faculty members who began work within the past year, a referral is not required. Their partners may complete the Dual Career request for services form to initiate services.

Requests on Behalf of Prospective Faculty Members

Referrals on behalf of prospective faculty members are welcome but are not required to initiate services. The partners of prospective faculty members who have been selected for an on-grounds interview, completed an on-grounds interview, or have an outstanding offer of appointment may directly complete the request for services form.
