AOR Vendor Self Review

Thank you for your interest in becoming a CVS Health approved Vendor. Please complete this form and submit it via the submit button at the bottom of the form. If your organization passes this review, the qualification process with our department will begin.

As follow up to this questionnaire and part of the onboarding process you will be required to submit documentation. The documents will include but will not be limited to a current audit or financial review statement. The information provided will be evaluated against predefined financial criteria and the results of your review will be communicated back to you via the Vendor Manager.

Please be advised that this process can take up to five months and if approved CVS conducts annual performance and financial reviews.
