(Platinum and Gold Chapters)

Please complete this form to request ASHE publications that were awarded to Platinum/Platinum Elite and Gold/Gold Elite status chapters as part of the ASHE chapter award program.


Once you complete and submit this form, your publication(s) will be shipped within 10 business days.


All publication requests are due by October 31, 2025. Requests received after October 31 will not be fulfilled.

Note: Only Chapters who participated in the 2024 Chapter Awards and received either Platinum/Platinum Elite or Gold/Gold Elite are listed. If your chapter name is not here, please contact Jenny Hull at jhull@aha.org.
