Licensee Course Review Request Form


To aid licensees in the selection of appropriate continuing education courses, the Board may review, without charge, a course brochure, outline, or agenda before the licensee attends the course, if the request is submitted at least 30 days before the course date. (COMAR 10.38.08.04 D)

Course Provider Continuing Education Application

​​Application Requirements:


Continuing Education applications must be received at least 6 weeks prior to the course start date.

Please include:

  1. A complete hour-by-hour course agenda.
  2. A clear and concise written statement describing the course's intended learning outcomes or instructional objectives.
  3. A curriculum vitae for each instructor describing the instructor's competence in the course subject matter and skill in instructional methodologies.
  4. Statement of the course's relevance to the clinical practice of physical therapy.

COMAR 10.38.08.04 A(5) If an instructor is not licensed by the Board and plans to perform physical therapy in conjunction with the course in the state of Maryland, an application for a restricted license must be obtained from the Board.

Please upload only 2 (two) PDFs with your application: one a combination of all your application/corresponding documents and the other PDF should be your payment receipt only.


  1. Complete hour-by-hour course agenda (total contact hours excluding registration, breaks and meals).
  2. Clear and concise written statement describing the course's intended learning outcomes or instructional objectives.
  3. Statement of the course's relevance to the clinical practice of physical therapy.
  4. Curriculum vitae for each instructor describing the instructor's competence in the course subject matter and skill in instructional methodologies.
  5. Course Payment Receipt: Payment Center - CE (course payment receipt is required for processing the application) **The payment receipt must be submitted as a separate attachment**
