- Complete hour-by-hour course agenda (total contact hours excluding registration, breaks and meals).
- Clear and concise written statement describing the course's intended learning outcomes or instructional objectives.
- Statement of the course's relevance to the clinical practice of physical therapy.
- Curriculum vitae for each instructor describing the instructor's competence in the course subject matter and skill in instructional methodologies.
- Course Payment Receipt: Payment Center - CE (course payment receipt is required for processing the application) **The payment receipts must be submitted as a separate attachment**
- Restricted License Payment Receipt: Payment Center - RL (payment receipt is required for processing a restricted license) **The payment receipts must be submitted as a separate attachment**
- Instructor's Active Home State Physical Therapy License: The instructor's home state license is required for processing a restricted license.
