Electronic Notarization of a

Paper Diploma:

This form is intended for alumni who wish to make a payment for the electronic notarization of a paper diploma via check or money order.


Note: Separate forms must be submitted for each degree/diploma.


Please follow these steps:


  1. Fill out the form completely.
  2. Ensure to upload a clear scan of your paper diploma and government-issued photo ID. Note: We cannot accept a download of your electronic diploma for this service
  3. Make your check or money order payable to the Trustees of the University of Pennsylvania.
  4. Mail your check or money order to the Office of the University Registrar (OUR).
