Please complete this form to request KYR Cards. This form will help us keep track of inventory. After completing this form, you will be notified if your request has been approved for pick up during the time and date indicated.

Our Office is open from 9am - 5pm Monday - Friday but our team works hybrid.

Due to safety concerns, you need to get an "Approval for Pick Up" notification before you can come to our office. Notifications will be sent to the email indicated. Requests are approved within 12 hours. Please check your spam folder if you do not hear from us within that time window.

Please note that KYR Cards will only be mailed out to those organizations in Central and Southern Illinois. Organizations in the Chicagoland Area will have to pick them up from our office.
