Due to disqualification from Hartnell College, you are requested to respond in detail to the following essay questions. Your status as a student at Hartnell College will be determined by your answers; therefore, it is important that you be specific and thorough.

Student Disqualification Appeal Acknowledgement*

I have read and understand the Hartnell College Academic Notice/Disqualification information (attached).

I will make an effort to regain good standing with the college.

I understand that if I am reinstated, I am required to earn a minimum 2.0 semester GPA and complete at least 50% of my units attempted. If I fail to do so, I will be subject to disqualification and sit out for one full semester (fall or spring).

If I am reinstated, I understand that I will be enrolled on a temporary status.

I have completed the disqualification appeal to the best of my ability and understand that if my appeal is denied, I can again appeal the decision according to the board policy.
