Unmanned Aircraft Systems (UAS) Request Form

Anyone wishing to operate a UAS or model aircraft on UO property or at a UO-sponsored event is required to obtain approval from Safety and Risk Services (SRS) prior to operation, and submit a completed UAS Request Form at least 14 days in advance of the requested flight date.


Third parties or vendors who seek to operate a UAS on UO property or at a UO-sponsored event will be asked to sign a "hold harmless" agreement and attest to insurance coverage in the amount required by SRS to cover UAS operations.


Required Attachments are:


  • Copy of remote pilot's license
  • Map of the flight plan and marked locations for UAS operators and time for take off. Please further include the number of pilot/drone monitors, and risk mitigation
  • FAA registration number
  • Oregon Department of Aviation registration number - only required if UO owns the UAS
  •  If UAS is not owned by the UO, a Certificate of Insurance (COI) is required. The COI must name the UO as "additional insured". To satisfy this requirement the "Description of Operations," shall include the following verbiage: "The State of Oregon, Board of Trustees of the University of Oregon, University of Oregon, and their respective officers, employees, and members."
  • If you are not UO-affiliated, download and fill out the Hold Harmless Agreement PDF and provide a copy of your COI
  • If a flight is planned within an FAA no-fly zone due to a campus event, a Waiver 107 is required. Risk Management must receive and approve this completed form before providing a confirmation letter for the requestor to submit to the FAA for final approval.



After submission, you’ll receive an acknowledgment of receipt from Safety and Risk Services, and possibly follow-up questions. You will receive approval or refusal of your request within 14 business days of submission of all of your required information.


Failure to attach all required documents may result in delaying approval of your request.

SECTION 2: PURPOSE OF UAS REQUEST/PROPOSED ACTIVITY

 

Provide full details of flight purpose (education, coursework, research, promotional, etc.), including identity of UAS operator(s). Depending on your intended use and activities associated with the use of your UAS, other university approvals may be required before you can operate your UAS on university property or at a university sponsored event. For example, any proposed videography, photography or recording may require approval from University Communications and may be subject to an Export Review by Sponsored Projects Services.

Please indicate what campus location the flight(s) will take place. Note the following:


UAS requests for Northeast PDX Campus (or locations near and around Portland) must obtain approval from the Port of Portland.


Departments wishing to fly drones outside of UO owned property must also obtain permission from the property owner and/or city, county, or federal agency.

 

In order to create a safe environment we ask that you identify exact locations for drone flight operator(s) and time of flights. Please use this space below to indicate for all flights include dates and times.

If you would like to request multiple flight locations and times please check the box below. If this is for a University of Oregon drone flying class. Please note you do not need to complete multiple location entries. Simply attach a list of your planned flights for the term. The list should include locations and proposed dates.

By typing my name below, the individual/entity submitting this request agrees to and will abide by all University policies and procedures governing the use of UAS on or over University property or a University sponsored event. They further agree that by electronic signature they are attesting to the truthfulness of the representations in the request form.


A copy of the approved UAS Request Form must be in possession of the operator at all times during the activity, and upon request must be presented to any University official or representative with control or jurisdiction over the activity.


The University reserves the right to request additional documentation as a condition of approval and operation. In addition, any operator violating any portion of the University’s UAS Policy or Procedure will be held accountable for their actions. The University further reserves the right to revoke previously approved UAS (drone) requests due to emergency, safety or federal air traffic control considerations.
