1. Describe the business process that needs to be addressed.
2. What metrics can be provided to help the change review board understand the scope of the requested change (Number of Students, Number of Transactions, Number of Hours, etc)
3. How is this issue currently being addressed on your campus?
4. How will you address the requirement if the change request is not approved? Is there a workaround?
5. Does this request have a financial impact (paycheck, benefits, retirement, budget, year-end, etc)
6. Is there an extenuating urgency associated with this request (Must be accompanied with any Priority of Production Critical)
7. Does the change impact state or federal compliance?
