Palliative Care Policies and Standards Intake Form (Nova Scotia Health Palliative Care Staff)

Intake Form Purpose: To identify opportunities and/or issues arising on the front lines of care, or through other channels (i.e., strategic priorities, Accreditation) that would benefit from the review, support and/or guidance of the Policies and Standards Working Group (a working group of the Palliative Care Network). The purpose of the working group is to provide leadership, coordination and accountability related to the identification, development, and maintenance of required policies, procedures, guidelines, protocols, and standards to support the palliative care delivery system.


Intended Audience of this Form: Any clinician or formal leader working in the palliative care system at Nova Scotia Health. Note: If you are a clinician, please ensure you have discussed this submission with you supervisor/manager before submitting.
