AALAS Training Request Form

All principal investigators (PI) and research staff performing animal procedures must complete two requirements prior to Animal Study Protocol (ASP) or addendum approval:


  • AALAS Online Training
  • Occupational Health Services (OHS) Program Enrollment

General Information

Please provide both your first and last name.

Please provide your Northwestern University (NU) email address. If you work with Lurie Children's, NM, or Shirley Ryan Ability Lab then that email address is also acceptable.


If you are transitioning from another academic institution and do not yet have an NU email, you may use your current institution's email address, but please contact the IACUC Office at acuc@northwestern.edu once you have an NU email address so that we can update our records.

Are you a new member of this laboratory?

If you are joining a Northwestern University laboratory and performing animal work for the first time, then please select "New." If you have already completed required training previously and are trying to identify specific courses to take, please select "Established."

Lab Information

Enter all species that you will be working with (e.g., mice, rats); also include "satellite" if you will be working in a satellite space.

Please enter the first and last name of your Principal Investigator (PI).

Please provide the Northwestern University (NU) email address for the individual that will approve your training request (i.e., your PI, Lab Manager, or Supervisor). It is also acceptable to provide an NM, Lurie Children's, or Shirley Ryan Ability email address if that is what your PI/Lab Manager/Supervisor uses.

Will you be working on a protocol that uses tamoxifen?*
Will you be working with human-derived materials in rodents?*
Will you be performing survival surgery?*

Additional training is required in order to perform survival surgery. If the only survival surgeries performed are Tail Biopsy on Mice Over 24 Days of Age and/or Toe Clipping, then some of the additional training is not required.
