Open Doors Program Application

 

Purpose of this Form

Use this form to submit a program application or revision for OSPI's approval. This includes


  1. Initiating a new Open Doors Program
  2. Changing or updating an existing program.


This form should be completed for any of the following:

  • Adding a pathway or ending a pathway
  • Changing of partnership status
  • Changing the delivery model
  • Starting or launching a program for the first time
  • Obtaining a qualification code for added/changed pathways.



 

 

How it Works

  1. Upload your completed application materials (below).
  2. OSPI will review uploaded application package.
  3. Upon approval, OSPI will generate and deliver to you the qualification codes needed to run an Open Doors Program.

If you are updating or changing an existing program, please enter your current Qualification Code(s) if you have them.

 
 

List the school building or host location. Examples: Edmonds CC, Bellingham HS, or Mason County

Each Open Doors program may have one person who is responsible for reviewing and approving student waivers. The person must be at the school or district.

To complete your application, the following program files are required:


  1. Contracted Services Agreement (signed by district and contractor)
  2. Letter of Intent (signed by district)
  3. Scope of Work (signed by district and contractor)
  4. Narrative
  5. Calendar
  6. Flow Chart
