ADRD Curriculum Approval Application
Any curriculum used for training purposes in licensed care settings in the state of Florida must be approved by The Department of Elder Affairs in accordance with the specific requirements for each care setting. Approved curricula are valid for three years from the date of approval.
ATTENTION:
- All file uploads must be labeled appropriately by indicating what their content contains to undergo the review process. If any mislabeled files are included in your submission, then your application will be deleted from our records without further correspondence.
- Curriculum submitted that bears obvious signs of being AI-generated will receive an emailed notice that the submission will be deleted from our records and you will need to re-apply. Content must be submitted in plain, understandable language appropriate for the training audience.
- No physical material, such as DVDs, cassettes, CDs, or other related material will be accepted. All curricula and supplemental material must be submitted electronically (video links, MP4 files, PDFs, Word documents, etc.).
The contents of this online application form are written in accordance with DOEA Form ADC/ADRD-002, DOEA Form ALF/ADRD-002, DOEA Form HH/ADRD-002, DOEA Form Hospice/ADRD-002, and DOEA Form ADRD-002.
