ADRD Curriculum Approval Application
Any curriculum used for training purposes in licensed care settings in Florida must be approved by the Department of Elder Affairs (DOEA) in accordance with the specific requirements for each care setting. Approved curricula are valid for three years from the date of approval.
ATTENTION:
- File Labeling: All uploaded files must be clearly labeled to reflect their content. Submissions with mislabeled files will be deleted without further correspondence.
- AI-Generated Content: If submitted content is identified as AI-generated, a notice will be sent via email, and the submission will be deleted from our records. The applicant will need to re-apply. All content must be written in plain, accessible language suitable for the training audience.
- Electronic Submissions Only: Physical materials, such as DVDs, cassettes, and CDs, will not be accepted. All curricula and supplemental materials must be submitted electronically (e.g., video links, MP4 files, PDFs, Word documents).
The contents of this online application form are written in accordance with DOEA Form ADC/ADRD-002, DOEA Form ALF/ADRD-002, DOEA Form HH/ADRD-002, DOEA Form Hospice/ADRD-002, and DOEA Form ADRD-002.
