Group GAP Starter Form

This form will ask specific questions needed for the GAP application and information about your current practices. Please submit this form by Friday, November 1st. There is no right or wrong answer about general practices. This is to customize your templates better. Be as honest as possible, even when you think your practices might not qualify.
