As part of our FY25 DEI Initiatives, we would like to take an inventory of all current and planned DEI efforts within the Department. This information will help us keep track of all DEI activities which we can then use to report to the School of Medicine, Executive Team and the larger Department. As you are submitting the efforts, please feel free to include all the names of people you know to be involved in each effort. If submitted on behalf of other members, please let them know to reduce multiple similar entries. Additionally, if you have multiple efforts/initiatives within one category, please submit separate forms for each effort/initiative.
