This form should be completed by a signatory contractor or a member who wishes to reschedule training. Approval will only be provided where there are extenuating circumstances.

Authority to Change

In signing a training contract, employers have entered an agreement, which binds each party to obligations under the Division of Apprenticeship Standards (DAS) and or the Department of Labor (DOL). In relation to attendance at off the job training, employers are obligated to:

  • Release the apprentice from work to attend any training and assessment specified in the training plan;
  • Ensure the training plan is followed, training records are kept up to date and the apprentice's progress is monitored.

I certify that I am a person within our organization authorized to seek a change to an apprentice’s Training Schedule and I understand:*
  • Our organization’s obligations as listed above.
  • If approved, rescheduling training may result in the student attending back to back training sessions.
  • The students rescheduled class is mandatory attend or cancel.
  • Rescheduled training may not be at the same facility as the initial training.

Training Date Option*

When applying for a reschedule, apprentices must understand that a rescheduled class date may be in the same/next month as classes previously assigned. All rescheduled class dates are mandatory attend or be canceled.

SWMSCTF will make every effort to accommodate your request; however, the date chosen may not be granted due to scheduling concerns.

Once the training has been rescheduled, please inform the apprentice of the rescheduled training date.
