Course Transfer Request Form

Coordinators: Please complete the following form to the best of your abilities. At the end of this form is a field to upload any course documents (syllabi, transcripts, course descriptions) that may help Graduate Studies adjudicate this request for course transfer.


The acceptance of credit earned at another institution and applied to an OHSU academic program at the same course level will be based on the quality of the institution from which the student transfers; assessment of the comparability and relevance to the OHSU program; grade received in each course; any articulation agreements between OHSU and another academic institution. Relevant credit may be accepted from any institution accredited by an accreditor that was recognized by the Council for Higher Education Accreditation or the U.S. Department of Education at the time the credits were awarded. Transfer credits are evaluated without regard for delivery method and include courses completed in-person or remotely, and synchronously or asynchronously.


Acceptance of credit in transfer is governed by OHSU policies 02-70-005 (Transfer of Course Credit) and 02-50-055 (Enrollment of Students in Multiple Degree/Certificate Offerings). Before authorizing the transfer of coursework, school officials are responsible for ensuring that the resulting transfer aligns will all policy requirements, including maximum transfer credits accepted and maximum age of transfer coursework.


For graduate programs, no more than 1/3 of credit hours toward degree requirements can be transferred from another accredited academic institution without prior approval of the dean and provost. For undergraduate programs, a minimum of 45 credits must be completed in residence at OHSU. In all instances, only earned credit can be transferred; grade point average (GPA) does not transfer.


Finally, all transferable credits must have been earned in the last 7 years. No courses completed >7 years ago are eligible for credit at OHSU.
