2024 GME Excellence and Innovation Awards
NOMINATION FORM INSTRUCTIONS
These awards recognize exemplary efforts to improve graduate medical education by a UCSF: resident, fellow, program administrator (or other GME administrative staff), or faculty member (including program directors, associate program directors, core faculty, and others).
The awards will be publicly presented on Monday, May 20, 2024, at a special GME reception (4:00pm–5:30pm at the Parnassus campus).
Eligibility/Selection Criteria:
Nominees will have demonstrated a commitment to advancing graduate medical education through educational and clinical quality improvement, service excellence, and innovation. Examples of excellence and innovation might include:
- Demonstrated educational leadership at UCSF or nationally
- Participation in innovative GME-related projects (i.e. curriculum, QI projects, assessment strategies, technology, etc.)
- Extraordinary efforts to address resident/fellow needs and well-being
- Extraordinary efforts to advance diversity, equity, inclusion and/or anti-oppression
- Other contributions to improving graduate medical education at UCSF or nationally
Recipients who have won the GME Excellence and Innovation Award within the last three years are not eligible to be nominated. View the list of past recipients.
Required Nomination Materials:
- Letters of recommendation: Two letters of support citing specific examples of the nominee's contributions to GME excellence and innovation related to the eligibility criteria. Letters should be no more than two pages long and in PDF format.
- Endorsement of nominee: Endorsement from the nominee's Supervisor and Department Head is required for the nomination to be considered. You do not need to submit written endorsements but you will need to confirm on this form that you have received the endorsement for the nominee.
- Completed nomination form: Nominations must be submitted via this form by the deadline below.
All nomination materials must be submitted by 11:59pm on Sunday, April 21, 2024.
