• Each institution may nominate one radiology resident who will be in their second year (PGY-3) at the time of the 2025 RSNA Annual meeting.
• Separate nominations from different residency programs will be considered

• A response must be provided for each of the questions.

• A signed letter or email from the applicant's Chair or Program Director must be emailed to dor@rsna.org addressing the questions within the form.
