Please complete this form to request access to student data within Campus Solutions. Once complete, the supervisor listed at the bottom of the form will be notified for approval. Once approved, your request will be sent to your campus security liaison for processing.

Please ensure that you select the proper campus so that your request is forwarded to the correct Security Liaison.

All individuals requesting access to student data must have completed the current-year FERPA Training within UMS Academy and passed the final assessment with a score of 80% or higher.


Proof of successful completion must be attached to this request as the campus security liaisons do not have access to your test results.
