2025 BoS PIT/HIC Submission Form

Please complete this form to submit your Point-in-Time Count and/or Housing Inventory Count reports to the NCCEH Data Center. You'll need to submit one form at a time for each HMIS participating project.

Emergency Shelter and Transitional Housing projects should upload:

D007 - 0630a Sheltered PIT

Permanent Supportive Housing and Rapid Re-Housing projects should upload:

D006 - 0628 HIC Supplement
