Cancellation/Refund Request Form

This form is used to request a cancellation of memberships, trips and workshops, personal training, small group fitness, and aquatic programming. This form can also be used to request a refund for any of the following services. Please check our policies page to see if your product or services is eligible for cancellation or refund.

Cancellation/Refund Information

If In-Person Credit Card Payment was made and you are approved for a refund, Recreation Services will contact you for your payment information to process the request. You must provide your information within 10 business days of the approval or the refund request will be denied.


A Cancellation/Refund Request Form must be completed and any applicable documentation must be provided to request a cancellation or refund.


A cancellation or refund is not guaranteed and will be based off the product, service, or program cancellation/refund policy, as stated on the Recreation Services Website.


Upon approval, a refund will be processed within 7-10 business days. Approved refunds may be processed on a prorated basis. Approved refunds will be refunded back via the original method of payment – with the exception of U-Bill & Payroll Deduct.
