A Cancellation/Refund Request Form must be completed and any applicable documentation must be provided to request a cancellation or refund.
A cancellation or refund is not guaranteed and will be based off the product, service, or program cancellation/refund policy, as stated on the Recreation Services Website.
Upon approval, a refund will be processed within 7-10 business days. Approved refunds may be processed on a prorated basis. Approved refunds will be refunded back via the original method of payment – with the exception of U-Bill & Payroll Deduct.
BY CHECKING THE BOX FOR THE CANCELLATION/REFUND REQUEST FORM, I STATE THAT I HAVE READ THIS NOTICE AND UNDERSTAND THAT ALL CANCELLATIONS AND REFUNDS ARE SUBJECT FOR APPROVAL AND WILL BE APPROVED BASED ON THE CANCELLATION/REFUND PRODUCT, SERVICE OR PROGRAM POLICY.
