EM Staff Time Off & Remote Day Requests



Please use this form to request vacation or personal time off (PTO), and remote day requests (remote day swap or additional remote days).


For sick time (PTS), please contact your supervisor directly. You do not need to complete this form.



***For exempt/non-exempt staff with PTO, you can also use these hours for wellness days so you can relax, maintain your physical/mental health, or take care of yourself/family members.

  • Vacation: Hours are accrued the last day of each month, after you have earned the hours for that month. Accrual rates are based on length of service and exemption status. Hours roll over from year to year, with a maximum accrual of 240 hours. (Prorated for part-time benefits-eligible employees)


  • Personal Time Off (PTO): Twenty four (24) hours of PTO are available at the beginning of the calendar year. Unused PTO hours do not roll over from year to year. (Prorated for newly hired and part-time benefits-eligible employees.)


  • Floating Holiday: For full time, benefits-eligible staff, 16 hours are available on Jan. 1 or on the date of hire after Jan. 1. Can be used on any day or partial day within that calendar year, which is mutually agreed upon by the employee and manager/supervisor. Unused floating holiday hours do not roll over to the next year.

For remote requests, I understand my request is only for a specific date or timeframe, and approval of such does not change my work status or hybrid schedule on file. I will also be responsible for updating my timecard accordingly.
