This form is to submit a request for a full review to the Secondary Data Use (SDU) Review Committee. It is intended for the contract officer to fill out, with input from the PI.

The SDU Review Committee covers Stanford School of Medicine, Stanford Health Care, and Stanford Children's Health, including their affiliates. It adjudicates how the three entities may use patient data with vendors, internal research, industry, and research partners.

The committee has established a framework to guide the sharing of Stanford health data with external collaborators. Stanford's secondary data use principles guide the ethical, responsible, and legal implications of secondary data use with our industry sponsors and vendors. Requests will be subject to adhering to the 12 Guiding Principles for sharing Stanford Health data with external collaborators. These are not yet available for broad distribution at this time.
