2026 School of Nursing Alumni Awards Nomination Form

Nominations are due by 11:59 p.m. September 14, 2025

OHSU alumni and community members are encouraged to submit nominations, which are reviewed and selected by the OHSU School of Nursing Alumni Association’s Awards Work Group and endorsed by the School of Nursing Alumni Association. Nominations remain active for three cycles. You can learn more about the 2025 award recipients here.

The 2026 awards will be presented in Portland, Ore. on Saturday, June 27, 2026.

Please note that this form cannot be saved and returned to once started.

Before beginning the form, please prepare a single .pdf file containing: the nominating statement; nominee CV; and up to two letters of support. Nominating statements and any optional supporting letters should be no more than 750 words in length.
